Apprentice travel request form

Apprentices or trainees who require air travel will need to complete this Travel Request form by clicking on the New booking request button. This needs to be submitted no less than two weeks prior to the date of the requested travel.

Changes to flights must be submitted by clicking on the Change to existing booking button.

Type of request

The completed form must be received by North Metropolitan TAFE no later than two weeks prior to commencement of the first block date of attendance.

  • Students who make changes to travel bookings for personal reasons are responsible for any costs associated with booking changes. By signing this request you are accepting the associated cost.
  • Employers who request a change to travel will be responsible for any costs associated with the change.
  • Where the change is initiated by North Metropolitan TAFE, and beyond the control of the student, NMT will accept the costs of the change however:
    Students who complete training two (2) days before scheduled flights are not eligible for a DTWD funded flight change. Students who complete training three (3) or more days prior to scheduled flight may be eligible for a funded change at the discretion of DTWD.

Students who complete training two (2) days before scheduled flights are not eligible for a DTWD funded flight change and will need to retain their original travel dates unless evidence of extenuating circumstances is provided.

  • Whilst every effort will be made to accommodate preferences, flights must be booked in accordance with State Government procurement guidelines which may affect flights that can be selected. Although a flight might be available at your preferred time, North Metropolitan TAFE is obligated to purchase flights that are best fare of the day unless reasonable cause can be demonstrated.
  • Students who complete training two (2) days before scheduled flights are not eligible for DTWD funded flight change. Students who complete training three (3) or more days prior to scheduled flight may be eligible for a funded change at the discretion of DTWD. In extenuating circumstances, a flight may be changed, however the apprentice or employer will incur the cost associated with the booking changes. Payment will be required prior to changes being made. All changes will require employer authority and must be requested by choosing 'Change to existing booking' as type of request on this form.
  • By signing this form, I consent to my personal details being disclosed to the travel agent and authorise North Metropolitan TAFE to process the travel booking(s) on my behalf.
  • Students who make a request for travel booking and do not make use of that flight may be asked to reimburse North Metropolitan TAFE for costs associated with the booking, unless extenuating circumstances can be demonstrated within 10 working days.
