Freedom of Information Access Application

To make a valid FOI access application you must give enough information to enable the requested documents to be identified.

Many fields in this form are optional. Please complete what you can to assist North Metropolitan TAFE to better understand what you are seeking and to deal with your application effectively and efficiently.

There is no application fee if you are seeking your personal information only. There is a $30 fee payable for an application for non-personal information.

The fee may be refunded when a payment is made in error, or if you withdraw your application. Refunds are processed back to the orignal payment source with 7 days of approval. Please contact the receiving agency to commence the refund process.

If you lodge your application without the required fee, your application will not be valid until the fee is paid.  North Metropolitan TAFE is required to let you know if your application is not valid because you have not paid the required fee.



North Metropolitan TAFE may also impose charges for a non-personal information application but only as set out in Schedule 1(opens in a new tab)  to the Freedom of Information Regulations 1993.

Third party information

If you have made an application for non-personal information, you may choose for third party personal or commerical/business information to be excluded from the scope of your FOI request. Excluding third party personal or commercial/business information may mean that the agency is able to deal with your application in less time.

Under sections 32 and 33 of the FOI Act, the agency must take steps to seek the views of any third parties whose personal or commercial/business information it proposes to disclose.

Excluding documents or information you do not need
It may be helpful to exclude certain documents or information from your application if it is not what you really want. Excluding particular information may help the agency deal with your application more quickly by reducing the number of documents to be located.

By submitting this form, you are providing your personal information to North Metropolitan TAFE.Your personal information will be used for the purpose of processing your request.

North Metropolitan TAFE may consult with third parties in considering whether an exemption under the FOI Act applies to the documents that you are requesting. This may involve disclosing your name, the scope of your request, and the documents falling within the scope of your request that concern the relevant third parties.

If necessary, North Metropolitan TAFE may transfer your request under section 15 of the FOI Act to another agency. You will be advised if this happens.
