Academic Records Request

Once you've completed your studies, you are entitled to receive your award/qualification i.e.

  • Certificate,
  • Diploma,
  • Advanced Diploma, OR
  • Statement of Attainment (for skill sets, or individual units completed)

You can also request an Academic Record at any stage of your studies. Students can access their results and progress via the student portal(opens in a new tab)

Unique Student Identifier (USI)

Please note that any documentation requested via this application form cannot be issued if you haven’t provided your USI and provided permission for North Metropolitan TAFE to be able to access your USI.

  1. Please go to www.usi.gov.au to create or retrieve your USI. 
  2. Then follow the instructions to provide North Metropolitan TAFE with permission to view your USI transcript. 
  3. Or contact Student Services for further information on 1300 300 822.

Apprentice or Trainee
  • You do not need to request your award
  • If you believe you have completed all contract requirements yourself or your employer will need to contact our Apprenticeship Management Team to have your contract completed

  • Once your training contract has been signed by all required parties, your award will be issued. Please ensure the postal address listed on your training contract is correct to allow your award to be delivered successfully.
Current Student

You are currently finishing the last units in your qualification:

  • For most students completing their course, award issuance occurs once the final result has been entered into the Student Portal
  • Please note that the Student Portal is different from Blackboard, and final unit results must be recorded in the Student Portal for the award to be issued.
  • Results can take up to two weeks to be finalised, once finalised your award will be posted. Once dispatched, delivery typically takes between five and ten business days.
  • Awards are sent to the postal address listed in your Student Portal. Postal tracking is not available, it is important your address is current to ensure accurate delivery. If your award was issued before you updated your address, a re‑issue fee will apply. 

 


 

Previous Students

Re-issue fees: If your Award and Record of Achievement has already been issued you will be required to make one of the following payments.

  • Award and Record of Achievement $50: to receive your award certificate and Record of Achievement showing units of competency completed within course
  • Record of Achievement $20: receive the transcript of units you completed within the qualification

Studies completed before 2018 may be held at either North Metropolitan TAFE or South Metropolitan TAFE

  • I have applied for Credit Transfers and provided evidence from another registered Training Organisation (RTO) to North Metropolitan TAFE for incomplete units.
  • I have completed all of the units required to complete the qualification being applied for.
  • I have logged on to the Student Portal and confirm that my name, address and contact details are up-to-date.
  • All fees and charges for all enrolments have been paid in full.

  • My training contract has been signed by myself and my employer and delivered to the Apprentice Management Team
